Otto is the younger brother of Lord Hobert Hightower, a powerful lord of the Dominion, and standard-bearer of House Tyrell. At some point in his life, Otto met and married Lady Alyrie Florent, with whom he had several children, including Gwayne and Alicent Hightower. Otto and his wife Alyrie moved to King's Landing to join the court of King Jaehaerys I Targaryen. They took their daughter Alicent with them, but left their eight-year-old son, Gwayne, in Antigua to be raised there, prompting his resentment towards his family. The Hand of the King and Prince of Dragonstone, Baelon Targaryen, passed away after a visceral breakdown Otto was widowed when his wife succumbed to illness.
