In the films, it is portrayed as a decaying and imposing structure, complete with long, foreboding corridors, barred windows, and disturbing patient rooms. The front door is ominously marked "Death Awaits" in spray paint. The hospital serves as the primary setting for the paranormal investigation conducted by the Grave Encounters team, and the follow-up investigation from Alex's team. As the investigations continue, it appears as if the building takes up a mind of its own. The building changes and expands around itself, trapping both teams in what appears to be an infinite loop of the hospital. Doors vanish, corridors appear where there shouldn't be any, and Alex's team even manages to escape the hospital and end back at their hotel, only to reappear at the hospital after disembarking off the hotel elevator.
